We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's Social Work - OP & PD Team in North Lincolnshire. As part of a dynamic team, you will work collaboratively with service users, their families, and other professionals to assess needs and develop tailored care plans.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals' needs and risks.
  • Developing and implementing person-centered care plans in collaboration with service users.
  • Providing ongoing support and review of care plans to ensure effectiveness and appropriateness.
  • Working closely with multi-disciplinary teams and external agencies to coordinate care and support services.
  • Maintaining accurate documentation and case records in compliance with statutory requirements.

To be successful in this role, you will need the following essential qualifications and skills:

  • A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registered with Social Work England.
  • Proven experience in adult social work, particularly with older adults and individuals with learning disabilities.
  • Strong assessment and analytical skills, with a commitment to person-centered practice.
  • Excellent communication and interpersonal skills, with the ability to build rapport with service users and their families.
