We are seeking a dedicated and compassionate Social Worker to join our Adult and Community (A&C) team in Leicestershire, specializing in Cognitive and Physical Disabilities. This is an exciting opportunity for a qualified professional to make a real difference in the lives of individuals facing challenges due to cognitive and physical disabilities. You will work collaboratively with clients, families, and multidisciplinary teams to assess needs, develop care plans, and facilitate access to appropriate services and resources.
Key responsibilities include:
Conducting comprehensive assessments of clients' needs and strengths. Developing and implementing personalized care plans in collaboration with clients and their families. Providing ongoing support and advocacy to individuals and their families. Working closely with health and social care professionals to coordinate services. Maintaining accurate and up-to-date client records and documentation. Participating in training and development opportunities to enhance professional skills.The ideal candidate will possess the following qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Demonstrated experience in working with adults with cognitive and physical disabilities. Strong interpersonal and communication skills, with the ability to build rapport with clients. Excellent organizational skills and the ability to manage a caseload effectively.
