Join our dedicated team in Leicestershire as a Qualified Social Worker specializing in Adult Social Work within the Physical Disabilities (PD) Team. In this role, you will collaborate with a multidisciplinary team to assess needs, develop care plans, and ensure the delivery of high-quality support services.
Your key responsibilities will include:
Conducting comprehensive assessments of service users to identify needs and strengths. Developing, implementing, and reviewing person-centered care plans in collaboration with individuals and their families. Providing ongoing support and advocacy for service users, ensuring their voices are heard and respected. Working closely with other professionals and agencies to coordinate services and promote best practices. Maintaining accurate and timely records in accordance with statutory requirements and organizational policies.To be considered for this role, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Registration with Social Work England. Demonstrable experience in adult social work, particularly with individuals with physical disabilities. Strong assessment, communication, and interpersonal skills.
