We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's Social Work - OP & PD Team in North Lincolnshire. This role offers a fantastic opportunity to make a meaningful impact on the lives of adults with learning disabilities and older people, ensuring they receive the support and services they need to lead fulfilling lives. As part of a dynamic team, you will work collaboratively with service users, their families, and other professionals to assess needs and develop tailored care plans.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing person-centered care plans in collaboration with service users. Providing ongoing support and review of care plans to ensure effectiveness and appropriateness. Working closely with multi-disciplinary teams and external agencies to coordinate care and support services. Maintaining accurate documentation and case records in compliance with statutory requirements.To be successful in this role, you will need the following essential qualifications and skills:
A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Proven experience in adult social work, particularly with older adults and individuals with learning disabilities. Strong assessment and analytical skills, with a commitment to person-centered practice.
