We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Salford. This role is pivotal in providing high-quality social work support to adults with complex needs, ensuring their safety, well-being, and access to essential services. As part of our dynamic team, you will work collaboratively with healthcare professionals, families, and community resources to facilitate positive outcomes for our clients.
Your key responsibilities will include:
Conducting comprehensive assessments of adults in hospital settings to identify their social, emotional, and practical needs. Developing and implementing tailored care plans in collaboration with multidisciplinary teams. Providing ongoing support and advocacy for clients and their families throughout their hospital stay and transition to community services. Ensuring compliance with relevant legislation and safeguarding protocols while promoting the rights and dignity of individuals. Maintaining accurate records and documentation in accordance with organizational policies and procedures.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, MA in Social Work). Current registration with Social Work England. Experience working with adults in a hospital or health care setting. Strong assessment and analytical skills, with the ability to develop effective care plans.
