As a member of our Disability Team, your key responsibilities will include:

  • Conducting comprehensive assessments to identify the needs and strengths of service users.
  • Developing and implementing tailored support plans in collaboration with individuals and their families.
  • Providing ongoing support and guidance to help individuals access services and resources.
  • Working closely with multidisciplinary teams, including healthcare professionals and community organizations.
  • Maintaining accurate documentation and case records in compliance with local and national regulations.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS, or equivalent).
  • Registration with Social Care Wales.
  • Experience working with adults with disabilities or within a similar social work environment.
  • Strong communication and interpersonal skills, with the ability to engage effectively with diverse individuals.
  • Excellent problem-solving skills and a compassionate approach to client care.
