We are seeking a qualified social worker who is committed to fostering independence and advocating for the rights of those with disabilities.
As a member of our Disability Team, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of service users. Developing and implementing tailored support plans in collaboration with individuals and their families. Providing ongoing support and guidance to help individuals access services and resources. Working closely with multidisciplinary teams, including healthcare professionals and community organizations. Maintaining accurate documentation and case records in compliance with local and national regulations.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS, or equivalent). Registration with Social Care Wales. Experience working with adults with disabilities or within a similar social work environment. Strong communication and interpersonal skills, with the ability to engage effectively with diverse individuals.
