Your key responsibilities will include:

  • Conducting comprehensive assessments of adults with learning disabilities to identify their needs and preferences.
  • Developing, implementing, and reviewing care plans in collaboration with service users and their families.
  • Providing advice and support to individuals and their families regarding available services and resources.
  • Working closely with multidisciplinary teams to ensure integrated service delivery.
  • Maintaining accurate and timely records in accordance with legal and organizational requirements.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, CQSW).
  • Current registration with Social Work England.
  • Proven experience working with adults with learning disabilities in a social work capacity.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, demonstrating empathy and understanding.
