We are seeking a dedicated and compassionate Qualified Social Worker to join our Locality Team in Cornwall, focusing on adult services. This role is pivotal in supporting vulnerable adults within our community, ensuring their needs are met and their voices are heard. As part of our dynamic team, you will work collaboratively with service users, families, and professionals to promote wellbeing and safeguard individuals at risk.

Your key responsibilities will include:

  • Conducting comprehensive assessments of service users' needs and risks.
  • Developing and implementing care plans in partnership with individuals and their families.
  • Providing ongoing support and advice to service users, ensuring access to necessary resources and services.
  • Collaborating with other agencies and professionals to coordinate care and support.
  • Maintaining accurate and up-to-date case records in compliance with local policies and legislation.
  • Participating in team meetings and contributing to the continuous improvement of services.

The ideal candidate will possess the following qualifications and skills:

  • A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registered with Social Work England.
  • Proven experience in adult social work, ideally within a locality team setting.
  • Strong assessment and analytical skills, with the ability to make informed decisions.
  • Excellent communication and interpersonal skills, fostering positive relationships with service users and stakeholders.
  • Ability to manage a diverse caseload effectively and work under pressure.
