Social Worker - Leaving Care - Enfield
We are seeking a passionate and dedicated Social Worker to join our Leaving Care team in Enfield. This role is pivotal in supporting young individuals transitioning from care to independent living. As a Social Worker, you will work collaboratively with young people, their families, and various stakeholders to ensure that each individual receives the support and guidance they need to thrive in their new environment.
Your key responsibilities will include:
- Conducting comprehensive assessments of young people's needs, strengths, and aspirations.
- Developing and implementing tailored support plans that promote independence and well-being.
- Providing ongoing emotional and practical support to young people as they navigate the challenges of leaving care.
- Collaborating with external agencies, educational institutions, and community resources to facilitate access to essential services.
- Maintaining accurate and up-to-date case records, ensuring compliance with relevant legislation and policies.
To be successful in this role, you will need to possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Experience working with young people in a leaving care or similar setting.
- Strong communication and interpersonal skills, with the ability to build rapport with young individuals.
- Excellent organizational skills and the ability to manage a diverse caseload effectively.
