We are seeking a dedicated and compassionate Qualified Social Worker to join our Preparing for Adulthood Team in Bromley. This role is pivotal in supporting young adults with disabilities as they transition into adulthood, ensuring they receive the guidance and resources necessary to lead fulfilling lives. You will work collaboratively with individuals, families, and multidisciplinary teams to promote independence and empower clients in their journey towards adulthood.

Your key responsibilities will include:

  • Conducting assessments and developing person-centered support plans for young adults aged 16-25.
  • Providing direct support and advocacy to clients and their families, ensuring their voices are heard in decision-making processes.
  • Coordinating with other professionals, including health care providers, educational institutions, and community organizations, to facilitate smooth transitions.
  • Monitoring and reviewing progress, adjusting support plans as necessary to meet evolving needs.
  • Maintaining accurate and up-to-date case records in compliance with local authority policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
  • Registration with Social Work England.
  • Experience working with young adults with disabilities and knowledge of relevant legislation and best practices.
  • Strong communication and interpersonal skills, with the ability to build rapport and trust with clients and their families.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
