Torbay Council is seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Project Team. Key responsibilities include:
Conducting comprehensive assessments to identify the needs and strengths of service users. Developing and implementing personalized care plans in collaboration with service users and their families. Providing ongoing support and advocacy to help individuals navigate social care services. Working closely with multidisciplinary teams to ensure integrated care delivery. Maintaining accurate and timely records in accordance with local policies and legal requirements.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Demonstrable experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills to effectively engage with diverse service users.
