We are seeking a dedicated and compassionate Social Worker to join our Adults Community team in Wirral. As a Social Worker, you will play a vital role in assessing client needs, developing care plans, and providing support to individuals facing various challenges.
Your key responsibilities will include:
Conducting assessments to identify the needs and strengths of clients. Developing and implementing personalized care plans in collaboration with clients and their families. Providing ongoing support and advocacy to clients, ensuring their voices are heard. Collaborating with other professionals and agencies to coordinate services and resources. Monitoring and reviewing client progress and adjusting care plans as necessary. Maintaining accurate and confidential records in compliance with regulatory standards.To be considered for this position, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with the Health and Care Professions Council (HCPC).
