We are seeking a dedicated and experienced Advanced Practitioner to join our Hospital Adults team in Warrington. This pivotal role involves working closely with adults in a hospital setting, ensuring that they receive the highest quality of care and support. As an Advanced Practitioner, you will lead and mentor a team of social workers, collaborating with healthcare professionals to assess and address the complex needs of patients. Your expertise will significantly contribute to improving outcomes for individuals and their families.
Key responsibilities include:
Conducting comprehensive assessments to identify the social, emotional, and practical needs of adult patients. Developing and implementing care plans in partnership with patients, families, and multidisciplinary teams. Providing supervision and support to junior social workers, fostering their professional development. Ensuring compliance with relevant legislation, policies, and best practices in social work. Advocating for patients' rights and access to services, including community resources and support networks.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England (SWE). Significant post-qualification experience in adult social work, particularly in a hospital or health-related environment. Strong assessment and analytical skills, with the ability to make sound decisions under pressure.
