We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Sunderland. This dynamic role offers the opportunity to work within a supportive environment, making a significant impact on the lives of adults requiring social care services. As part of a multidisciplinary team, you will be responsible for assessing and addressing the social needs of patients, ensuring they receive comprehensive care during their hospital stay and upon discharge.
Your key responsibilities will include:
Conducting thorough assessments of patients’ social needs and risks. Developing and implementing care plans in collaboration with healthcare professionals. Providing emotional support and guidance to patients and their families. Facilitating access to community resources and services post-discharge. Maintaining accurate and timely documentation in accordance with policies and procedures.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Proven experience in adult social work, preferably in a hospital setting. Strong communication and interpersonal skills, with the ability to work collaboratively in a team.
