Join our dedicated Community Team in Devon as an Adult's Social Worker, where you will play a vital role in improving the lives of vulnerable adults within our community. As an integral member of our team, you will assess needs, develop care plans, and provide ongoing support to individuals and families. Your work will directly contribute to fostering independence and promoting well-being among service users.
Key responsibilities include:
Conducting comprehensive assessments to determine the needs of adults requiring support. Developing and implementing tailored care plans in collaboration with service users and their families. Providing emotional and practical support, ensuring individuals have access to necessary resources and services. Working closely with multi-disciplinary teams, including healthcare professionals, to ensure holistic care delivery. Maintaining accurate records and documentation in accordance with statutory regulations and organizational policies.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience working with adults in a social care environment. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
