We are seeking a dedicated and compassionate Adult Social Worker to join our dynamic team within a hospital setting in West Sussex. This is an exciting opportunity to make a difference in the lives of adults requiring social care support. The successful candidate will work collaboratively with healthcare professionals to assess patients’ needs, ensuring they receive the appropriate services and support during their hospital stay and upon discharge.
Key responsibilities and duties include:
Conducting comprehensive assessments of patients’ social, emotional, and practical needs. Developing and implementing care plans tailored to individual circumstances. Providing information and advice to patients and their families regarding available services and resources. Collaborating with multidisciplinary teams to ensure holistic patient care. Advocating for patients’ rights and preferences in care decisions. Monitoring and reviewing care plans to ensure effectiveness and compliance with statutory requirements.Essential qualifications and skills for this role include:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Proven experience in adult social work, preferably within a hospital or healthcare setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Excellent assessment and analytical skills.
