We are seeking a dedicated and compassionate Permanent Social Worker specializing in Mental Health to join our dynamic team in Tameside. In this rewarding role, you will play a crucial part in supporting individuals facing mental health challenges, ensuring they receive the necessary care and support to improve their quality of life. You will work collaboratively with service users, families, and multidisciplinary teams to deliver comprehensive assessments and tailored intervention plans.
Your key responsibilities will include:
Conducting thorough assessments of individuals’ mental health needs and risks. Developing and implementing personalized care plans in collaboration with service users and their families. Providing ongoing support and guidance to individuals, promoting recovery and independence. Working closely with healthcare professionals, community organizations, and local authorities to facilitate access to services. Maintaining accurate and up-to-date case records and documentation in compliance with local and national standards.To be successful in this role, you will need the following essential qualifications and skills:
A recognized Social Work qualification (Degree in Social Work or equivalent). Registration with Social Work England. Proven experience in mental health social work or related field. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
