Trafford is seeking a qualified Social Worker to join our Adults Community team in Altrincham. In this pivotal role, you will be responsible for assessing and supporting individuals, ensuring they receive the necessary services to enhance their well-being and quality of life.
As a Social Worker in our Adults Community team, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of adults in the community. Developing and implementing tailored care plans in collaboration with service users and their families. Providing ongoing support and guidance, advocating for clients’ rights and access to services. Collaborating with multidisciplinary teams and external agencies to ensure holistic care. Maintaining accurate and up-to-date records in accordance with legal and organizational requirements.The ideal candidate will possess essential qualifications and skills, including:
A degree in Social Work or equivalent qualification recognized by Social Work England. Current registration with Social Work England. Proven experience in adult social care, preferably within a community setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
